2025 Benefit Highlights:
- Fixed co-insurance on durable medical equipment on most plans (which means your clients don’t need to meet their deductible first)
- Fixed co-pays for labs and x-ray services on most plans
- Virtual visit benefits with $0 copay for virtual appointments for non- HSA and Standard plans
- Option to add dental with Momentum Dental on all Non- HSA plans-(reminder no waiting periods regardless of previous coverage)
- Vision benefits include material coverage with a $100/ year benefit for glasses or contacts- Illinois and MN- vision is included- WI members receive the adult vision if they purchase the dental coverage.
- New 6-Tier pharmacy design, with $0 cost meds for Tier 1
- Members with chronic conditions can receive self-monitoring tools at no extra cost
- Safe Harbor medications and supplies for $0 cost on our HSA plans
- Additional direct only plans available on www.Quartzbenefits.com for clients that don’t qualify for a tax credit, in addition to on exchange plans. Plans vary by state.
Network Updates:
- New! Quartz Gundersen Performance (WI) will be a new narrow network available for individual and families who live in La Crosse, Jackson, and Trempealeau counties only
- The same providers as Quartz One Achieve network-except the below health Systems are not part of the Gundersen Performance network. Tomah, Vernon, Oakleaf, Black River, Krohn, and ProHealth facilities-(reminder only for residence in the 3 counties listed above)
- ProHealth will no longer be in network effective 1/1/25 for Individual and family plan members.
Wellness Programs:
- Member well-being program, where clients can earn points for engaging in health activities and redeem them for rewards.
- Member Mental well-being program. Clients can use digital self-help tools and one-on one coaching and therapy appointments.
- For Qualified members, it’s an obesity and Type 2 diabetes reversal program, powered by Virta Health- This program helps members control blood sugar, reach an ideal weight, and reduce the need for certain medications.
