CenterWell and Conviva Ending Agent Care Coordination Program

Agility partners CenterWell Senior Primary Care and Conviva Senior Primary Care have informed us that the Agent Care Coordination (ACC) program will end effective July 31, 2025. This decision applies to all markets and brokers.

According to Cheri Greenfield-LaTour, SVP and Chief Clinic Operations Officer at CenterWell, they “are deeply appreciative of your role in making the ACC program successful, and your work in coordinating care by scheduling initial visits for your clients – after they have entrusted their primary care to a CenterWell or Conviva provider – has improved engagement and contributes to the overall satisfaction and health outcomes of your clients.”

Cheri continues in her statement, saying, “Brokers have been, and will continue to be, important partners in ensuring seniors receive comprehensive care that improves health outcomes and their quality of life. We are in the process of launching new internal capabilities – including new scheduling options – that will continue to increase and improve patient engagement, and you will receive additional information about these initiatives and how they will improve the delivery of healthcare for your clients.”
